SUMMARY:  Supports the Virtua Rowan School of Osteopathic Medicine, Department of Family Medicine’s quality goals:

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • The role of Lead Population Health Medical Technician performs patient outreach via MyChart, phone and mail to gather information and test results for various medical screenings.
    • Contributes to the achievement of excellence in health care to fulfill the mission of the Department.
    • Exhibits strong customer service skills in daily interaction with the public, patients, staff and physicians in the performance of job duties
    • Obtains copies of records from outside medical providers via fax request or physician portal access.
    • Ensures medical documentation is properly indexed into patient charts to close gaps in care.
    • Maintains updated spreadsheets of open gaps for 5 office locations.
    • Directs work study and work assistant personnel in assisting with gap closure.
    • Meet with residents on their Population Health rotation to review the work of the quality team and how the resident can assist these efforts in their practice.
    • Attend VMG quality meetings for information regarding specific areas to focus on each quarter.
    • Contribute to bi-monthly Family medicine quality meetings to discuss progress and barriers.
    • Create calling scripts for self and student use.
    • Create orders in Epic system to be given to patients.
    • Maintain contact with the student run cancer screening project, supplying patient information and sending out orders.
    • Provide gap lists to offices if needed.
    • Reports any needed supplies to appropriate person.
    • Liaise with VMG quality team members for gap closure questions.
    • Upload PDF files to Insurance portals for Quality initiative programs, i.e. Optum.
    • Provide patient education regarding self-management
    • Serves as a primary resource to clinical staff to establish quality goals using reports.
    • Participates in department's Performance Improvement activities.
    • May assist in supporting clinical operations with Family Medicine and Employee-Student Health based on needs of the department.
    • Performs other various duties and assignments as assigned.
  • Understands and adheres to legacy Rowan SOM compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.

EDUCATION AND/OR EXPERIENCE: 

  • Graduation from an accredited program in Medical Assisting with a current certificate.
  • At least one (1) year of experience in a medical office setting, preferably in a medical office or clinic setting required.
  • A good working knowledge of computers is required, to include creating and uploading PDF files, etc.
  • The ability to work independently, excellent communication skills, organizational skills, and flexibility is required.

PHYSICAL DEMANDS: 

Must be able to stand and sit for long periods of time.  Must be able to communicate very well with patients via telephone.
